Kadlec Clinic has an exciting opportunity for a Ambulatory Clinical Pharmacist to join our growing Primary Care team located in South Richland, Washington !
Shift Details: Full time, 1.0. Day shifts.
Graduation from an accredited College of Pharmacy required.
Post-graduate year 1 residency (PGY1) or three (3) years ambulatory pharmacy experience required.
Current Washington State Pharmacist Licensure required.
BPS Board Certified Ambulatory Care Pharmacist required within 2 years from date of hire into the position.
Graduation from an accredited residency program preferred.
PGY2 Ambulatory Residency preferred.
Duties include, but are not limited to, the following:
As an Ambulatory Clinical Pharmacist you will be accountable for addressing medication needs, developing sustained partnerships with patients, and practicing in the context of family and community. This is accomplished through direct patient care and medication management for ambulatory patients, long-term relationships, coordination of care, patient advocacy, wellness and health promotion, triage and referral, and patient education and self-management. The ambulatory care pharmacists may work in both an institutional and community-based clinic involved in direct care of a diverse patient population.
Provides Clinical Pharmacy Services in Kadlec Clinic for patients who are referred by a primary care provider or as part of team-based care protocol according to Clinical/Collaborative Pharmacy Agreement guidelines.
Collaborates with providers and caregivers to provide coordinated, comprehensive, high-quality care to patients within the Kadlec Clinic primary care setting.
Supports transition care management for high-risk populations.
Provides education to caregivers (providers and/or others) in written and verbal formats to continually advance the quality of care delivered in Kadlec Clinics.
Supports initiatives to improve patient safety and stewardship of healthcare resources.
Supports and initiates regulatory compliance opportunities within the various clinics of work.
