Kadlec Pharmacist Resident - PGY1 in Richland, Washington

Note: All applicants must have a Pharm.D. degree from and accredited school of pharmacy and be eligible for pharmacy licensure in the state of Washington. In addition to this application, all application materials must be submitted through the PhORCAS application system managed by the American Society of Health-System Pharmacists (ASHP). Kadlec has an exciting opportunity for a Pharmacist Resident to join our growing Pharmacy Residency team located in Richland, Washington!

Shift Details: Full-Time, 1.0, 8 hour variable shift

Qualifications:

Minimum Requirements:

  • Graduation from an accredited College of Pharmacy.

  • Current Washington State Pharmacist Licensure required.

  • Pharmacy Intern License may substitute for Pharmacist within the first 90 days of employment with a licensed Pharmacists’ supervision.

Preferred Qualifications:

  • PharmD

  • Experience in hospital pharmacy practices as a student, intern or pharmacist.

Duties include, but are not limited to, the following:

As a Pharmacist Resident you will participate in a training program which helps to develop the skills, knowledge and attitude necessary for the provision of high quality pharmaceutical care; Demonstrates the ability to function effectively as a staff pharmacist, meeting the same standards of performance, duties and functions; and performs all duties in a manner which promotes team concept and reflects the Kadlec's mission and philosophy.

  • Demonstrate and sustain compliance with the Caregiver Expectations and Code of Conduct.

  • Attentively supports daily pharmacist activities.

  • Perform assigned data base maintenance tasks within pharmacy information systems – examples may include Horizon Meds Manager, Connect Rx, Pharmagistics, Pyxis, Med Carousel, and STAR Financial.

  • Review and interprets physicians’ orders to assure that the drug, dosage form, and dose is appropriate for the patient’s age, weight, condition and co-morbidities. Checks for drug incompatibilities and allergies, and takes appropriate action when necessary.

  • Perform Narcotic Safe inventory maintenance tasks per management directives.

  • Perform and models excellence in pharmacist work.

  • Embrace a culture of professionalism and excellence in providing pharmaceutical care.

  • Provide prescribed medications and other pharmaceuticals as needed for patient care according to professional standards and practices.

  • Monitor individual patient drug therapy for medication safety and appropriateness.

  • Monitor inpatient dose per patient renal function status.

  • Follow Aminoglycoside, Vancomycin, and Adult TPN prescriptive authority protocols.

  • Actively participates in KRMC’s Antimicrobial Stewardship program.

  • Provision of decentralized clinical services on inpatient care units is required.

  • Respond to questions from physicians and nursing staff and patients regarding medicines and pharmaceuticals.

  • Oversee daily drug distribution service activities.

  • Oversee controlled medication procurement, distribution, security, and documentation compliance daily.

  • Perform medication safety oversight activities throughout the Medical Center.

  • Supervise and checks work of the pharmacy technicians; may assist in interviews, selection and training of new technicians.

  • Fill prescriptions for take-home medications; packages and labels medications following established procedure.

  • Oversee sterile compounding service activities.

  • Review and validates computerized medication order entry work of physicians and other medical care providers.

  • Perform specialized order entry functions and maintains patient pharmaceutical care documentation within the clinical information system.

  • Advise and consult with patients and other health professionals regarding medication therapy indications, efficacy, adverse effects, safety recommendations, administration guidelines, and optimum utilization practice.

  • Participate in Pharmacy Department performance improvement activities.

  • Maintain current professional knowledge through continuing education activities.

  • May be required to drive a vehicle (as applicable) if driving record is acceptable, as established by the insurance industry and required by KRMC’s commercial auto/valet insurer.

  • May perform specialized administrative assignments such as attending designated meetings, inventory management activities, medication education activities, employee scheduling, various quality assurance activities, quality improvement team participation or clinical pharmacy project work.

  • Supervise daily operation of Automated Drug Dispensing Devices and medication carousel automation systems.

  • Monitor organizational medication safety compliance.

  • Document drug therapy clinical interventions, medication errors, and adverse drug reaction events per pharmacy operational protocol.

  • Participate in all the requirements of the ASHP approved pharmacy residency, such as:

  • Fully engaging in each assigned rotation.

  • Actively participating in learning modules.

  • Reporting and evaluating and being evaluated.

  • Turning in assignments.

  • Completing necessary projects in a timely fashion, including major project.

  • Presenting findings at pharmacy meetings, including ASHP and Residency Conferences.

  • Abiding by standards and expectations set forth by ASHP and KRMC and KC.

  • Participating in staffing requirements.

  • Professionally supporting the advancement of pharmacy services.

  • Enhance medication safety and security through active monitoring and reporting of outliers or events related to safe and proper use of medications.

  • Participate in recruiting for continued residency program needs.

  • Foster academic learning by precepting student intern/extern pharmacists.
